Position Summary:

Responsibilities:
• Deliver on-site rounding, primary care, and advanced wound care services.
• Manage complex wounds, skin integrity issues, and post-acute recovery cases.
• Lead chronic care management and preventative health initiatives.
• Collaborate with physicians, nurses, and interdisciplinary care teams.
• Reduce hospitalizations through proactive, hands-on care delivery.
• Communicate with families, caregivers, and facility leadership.
• Maintain accurate documentation, medication management, and lab coordination.
Qualifications:
• Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
• Experience in geriatrics, wound care, primary care, or post-acute care preferred.
• Valid driver’s license and ability to travel within assigned territory.
• Strong desire to grow clinically and work independently.
• Passion for delivering high-quality, relationship-based senior care.
