· Initiate personalized plans of care with input from the individual and family based on the desired outcomes, personal preferences and available choices of the individuals being served.
· Utilize available resources in combination with other formal and informal supports and services to meet the needs of the individuals.
· Attend monthly, quarterly and annual contact (via phone or face-to-face) with individuals and families to discuss progress toward achieving goals identified on the support plan.
· Initiate and maintain documentation of notes and progress reports.
· Work as a team to assist individuals and their families in coordinating, selecting, and taking ownership of their plan.
· Continue learning about relevant, local accessible services and providers.
POSITION REQUIREMENTS:
· Bachelor’s Degree or higher in any field
· Bilingual required (Spanish)
· One or more years of experience working and/or caring for adults with disabilities;
· Computer Proficient (MS Office Outlook, Word, Excel, PowerPoint);
· Able to solve problems independently, effectively and creatively;
· Excellent Organization and Communication skills;
· Excellent time management skills;
· Must have a flexible schedule to meet the needs of our clients;
· Valid Driver’s license and car to be able travel;
· Complete on the job and required state training.
· State, Federal Criminal Background Checks and Central Registry check is mandatory.
· Successful completion of training required by the Division before rendering services.
