Position: Program Supervisor

Full‑Time/Part‑Time

Location: West Sayville, NY

Days/Hours: Tuesday - Saturday; 8am - 4pm

Description

Concern Housing is seeking a Program Supervisor to be responsible for the overall supervision and management of the program site. The Supervisor is responsible for managing the residence in a manner that promotes client growth, safety, and the effective delivery of rehabilitation services. In addition, the Supervisor oversees operations so that they maintain compliance with all program requirements.

Duties and Responsibilities
  • Assist with the supervision and coordination of all direct care services.
  • Primary responsibility for supervision and training of assigned staff, as well as on‑site training and supervision of all program staff.
  • Ensure that program and residence are in compliance with all regulatory guidelines regarding client care, safety and delivery of services.
  • Review closed‑out client charts upon discharge and ensure that all documentation is complete.
  • Act as a liaison with other service providers as well as with community agencies.
  • Assist with the coordination of staff schedules, including per diem staff.
  • Coordinate house meetings and liaise with Resident Counsel.
  • Perform periodic staff evaluations; participate in hiring and dismissal of staff.
  • Participate in weekly supervision meetings with the Program Director.
  • Assist staff and clients with issues related to securing and maintaining entitlement income.
  • Provide flexibility in scheduling according to program needs.
  • Assist Program Director with maintaining program occupancy rates including, but not limited to, the intake process, collection of supporting documentation and admission process.
  • Maintain compliance with NYSOMH, LIHTC, AHP regulations and other regulatory requirements.
  • Conduct weekly supervision with staff including weekly client review and ongoing chart review.
  • Monitor client charts for timely submission of paperwork, and qualitatively review progress notes, services plans and reviews, functional assessments and other required documentation.
  • Periodically fulfill responsibilities of Service Coordinator as assigned and as needed during staffing turnover.
  • Cooperate with the Property Department regarding site maintenance.
  • Adhere to all policies and procedures as set forth in Concern’s personnel manual.
  • Provide crisis intervention including on‑call responsibilities.
  • Remain current with all Agency required compliance documents and trainings.
  • Perform other tasks as assigned by Director.
Qualifications

Masters Degree in Psychology, Social Work or related mental‑health field preferred with at least 3 years experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good leadership and supervisory skills, good verbal and written communication skills, and the ability to make sound judgments regarding client care.
